HIGH School and MIDDLE School
- ALL Physicals & Health History Forms must be turned into the Athletic Office or High/Middle School Nurse no later than one week prior to start of the season
- ImPACT Testing will take place within the first few weeks of practices. The Athletic Trainer or designiate will administer the test.

If you have already created a family account, please log into your account before registering for additional sporting activities. This will alleviate having to re-enter the family information.
Registration is a two-step process so please follow these instructions carefully:
Step 1 - Register your student online. By completing this registration your child will have an updated information sheet for ALL sports and student activities they wish to participate in during the school year. The choices you select for sports / activities can be changed prior to the beginning of each athletic season and during the school year for clubs / activities.
PLEASE NOTE: If this is the first time you are registering a student, upon completion of the form, you will be asked to set up a Family Account. By doing so, future registration forms will be pre-filled for your convenience by allowing you to make changes and complete any new fields that have been added. Once a registration form has been submitted, a confirmation page will be displayed indicating that Step 1 has been completed.
If you have another student to register for a sport / activity, please log into your account and then select the appropriate registration form so all your children are associated with one Family Account.
EVERY STUDENT NEEDS A HEALTH HISTORY UPDATE EVERY SEASON
Step 2 - Pre-Participation Physical Evaluation and/or Pre-Participation Physical Evaluation History Form
A Pre-Participation Physical is valid for 365 days. It must be on file with the high / middle school nurse prior to the start of each athletic season.
A - If the Pre-Participation Physical will expire prior to the first day of practice for the selected sport, a new physical, including the four-page Pre-Participation Physical Evaluation History Form, Physical Exam Form signed by your physician and Clearance Form which includes the Cardiac Assessment Development Module must be submitted to the Athletic Office or high school nurse during the enrollment period to complete Step 2.
B - If the Pre-Participation Physical expires any time during pre-season practice or during the season, a new physical must be obtained to continue participation in the sport. THIS IS MANDATORY. However, the parent still must complete the Health History Update Form at the beginning of each season and submit the original copy to the Athletic Office during the enrollment period to complete Step 2.
C - A Health History Update Form is required every 90 days (beginning of each season). This is a State mandated policy.

Before a student may participate in athletics or activities, the items below must be satisfied – NO Exceptions!
1. All students must complete the Interscholastic Contract/Agreement (available inside the appropriate online activity registration form)
2. All students must meet academic requirements set by the Warren Hills Board of Education and the NJSIAA academic requirements.
3. To be an active participant at Warren Hills you must be cleared by the Athletic Director’s office.
Note: If there is a valid current physical on file with the school nurse, you are only required to complete the Health History Questionnaire Form. Please ensure all required signatures are provided.
The dates on the four-page Pre-Participation Physical Examination Form must all be the same and be signed and stamped by your physician. The forms must be complete, including height, weight, vision, pulse and blood pressure. All abnormal findings must be explained.
The first page of the Pre-Participation Physical Examination Form must be signed by the parent and student. Explain all “yes” answers: include dates, length of symptoms and treatment. Please attach a second piece of paper if more room is needed. The date on the Pre-Participation Physical Examination Form must be the same date as the physical exam.
The Special Needs Supplemental Form must be completed and signed even if the student does not have a disability. Forms will be returned if any information is missing. Be sure to also read the information on the NJSIAA Steroid Testing Policy, Concussion Testing Information, Sports Related Eye Injury Brochure and Sudden Cardiac Death in Young Athletes Information. All located inside the activity registration form.
It is your responsibility to verify with the Athletic Office office if your physical is valid and current. All original forms must be returned to the Athletic Office one week prior to the start of the season. Be sure to also read the information on the NJSIAA Steroid Testing Policy, ImPACT Concussion Program, Concussion Management Policy, Eye Injury Fact Sheet and Sudden Cardiac Death in Young Athletes Information.
Transportation Guideline ... it is highly encouraged that all athletes utilize the bus for transportation to/from their competitions. However, there may be an occasion where alternative means is required. By registering participation online, parents may transport their own child upon written clearance from the coach. Permission is required for each occasion. However, when transporting child(ren) that is/are not the son/daughter of the driver, the Private Transportation Application is required to be on file in the Athletic Office no later than 24 hours prior to the event. This form may be found under the Forms tab and must be signed by all parties involved.
